Assessment when Administering Meds
Before administering any medication remember your 10 rights of administration (Right medication, dose, time, patient, route, assessment, reason, teaching, documentation and evaluation). ↓
Look at your patient's chart and identify diagnosis and correct reason for administering the med.
Look at the patient's medication history (any contradictions...same or different meds?)
Does he/she have any allergies to meds.?
Are they NPO?
Do they have N/V?
Any other reasons you need to hold the medication? Look at your labs, vital signs and physical assessment findings.
Does your patient have knowledge of their medication? If pt. has never heard of the medication you are about to administer then re-check medication hx and or ask their PCP.
Is your patient in any pain?
Does your patient have any other needs before administering the medication?

After administering any medication make sure you evaluation your patient. Was the medication effective? Do you need to re-assess or take vital signs again? Was there any adverse reactions or allergic reactions to the medication? Are there any intolerable side effects?
